Requirements:

As part of HSMG’s regulatory requirements, HSMG and its contracted providers (PCPs and Specialists) are required to conduct the following activities:

• Providers are required to notify HSMG with at least sixty (60) calendar days’ notice if they intend to terminate their contract with HSMG.

• HSMG is required to send notices to any impacted membership with at least sixty (60) calendar days’ notice
